Current Utilization and Research Status of Herbal Medicine Sipjeondaebotang for Anemia : A Scoping Review

Background and Objectives :
Anemia remains a significant global health concern, particularly prevalent among the elderly. Conventional therapies frequently yield suboptimal outcomes, prompting interest in alternative interventions. This studyexplored the clinical application and therapeutic potential of Sipjeondaebotang (SDT), a traditional East Asianherbal formula, in the management of various types of anemia.

Methods :
We conducted a scoping review based on the Arksey and O’Malley framework, adhering to PRISMA-ScR reporting standards. Six databases were systematically searched for human clinical studies involving SDT,excluding in vitro and animal research. A total of 16 studies met the inclusion criteria, comprising 9 randomized controlled trials, 2 controlled clinical trials, 2 case series, and 3 case reports, involving 863 patients in total. The research was primarily carried out in China, South Korea, and Japan.

Results :
SDT was associated with improvements in hemoglobin concentration across different anemia types. In all controlled studies, SDT outperformed control treatments in terms of hematologic response. In surgical patientswith anemia, SDT contributed to reduced perioperative blood loss, shortened recovery time, and decreased dependence on transfusions. Furthermore, adverse events occurred less frequently in the SDT groupscompared to controls.

Conclusions :
These findings indicate that Sipjeondaebotang may offer a beneficial therapeutic option for anemia byenhancing hematologic markers and supporting clinical recovery. Nonetheless, further rigorously designed clinical trials with larger sample sizes are required to determine standardized dosing regimens, treatment durations, and to establish its efficacy and safety across diverse anemia populations.
