The Western scientific world or rather Westernizing often criticizes acupuncture for not or not to satisfy thecriticisms of the Evidence Based Medicine.
The question can be posed in the other direction: what are the validities of our Western treatments with regardto EBM ?
Taking as a problem the spinal pain in the broad sense (neck pain and low back pain), this communication draws up a short inventory of the value of our treatments (physical, medicinal, and acupunctural) with regard to the meta-analyzes of EBM identified by the Cochrane Data Base of Sytematic Reviews.
We reviewed recent meta-analyzes for 15 common treatments proposed for spinal pain.
Those studies compare, drugs, manipulations, infiltrations, tractions, electrotherapy , exercices and acupuncture.
This overview of the EBM literature shows us that the lack of EBM evidence blamed for acupuncture is in factjust as valid for Western medicine.
This does not necessarily mean that we must abandon all treatment for our patients, but that we can on the contrary try an acupunctural treatment without shame because we are no less scientific than our non-acupuncturist colleagues and that, ultimately, only the result individual obtained with the patient counts and not the statistic.

Photobiomodulation (or Low-Level Laser Therapy, LLLT) is the process by which chromophores selectively absorb low-energy wavelengths to induce cellular signaling phenomena that trigger major biological synthesis and thus activate the cell regeneration process. Photobiomodulation has an effect on the cellular metabolism of our tissues comparable to photosynthesis in plant cells.
Light energy is transferred to the various organs of our cells to stimulate metabolic functions, which produce several remarkable clinical effects, such as analgesic, anti-inflammatory, and wound healing effects.
The mitochondria perform a vital function: cellular respiration. It is here that glucose completes its metabolism by encountering oxygen supplied by red blood cells. This encounter allows the cell to produce ATP, or adenosine triphosphate, a molecule that constitutes the cellular energy reservoir. This glucose metabolism allows the movement of protons and electrons, producing water and ATP.
To enable electrical currents to flow, the cell uses a chain of molecules, including Cytochrome C Oxidase, a protein containing iron and copper, hypersensitive to red and infrared light: as soon as a photon touches it, it orders the production of ATP.
The light therefore produces an immediate effect: activating the protein and delivering energy to the cell. While regional effects using wide-scanning devices have been studied for a long time, recent techniques allow for specific focusing on acupuncture points similar to laser or electrotherapy, which are very promising and have a perfect place in the acupuncturist’s therapeutic arsenal.

Combining acupuncture with a more localised technique is common in TCM (moxa, cupping, etc.), but fewcases have been reported using PBM.
This clinical case illustrates the fruitful synergy between the local regenerative action of PBM and the generaleffects of acupuncture, resulting in a marked improvement in epistaxis.
